If you are ordering ready-made puzzles from the Puzzles for Sale page, you can either (1) make your
purchase(s) directly from that page via PayPal and disregard this page, or (2) complete this form, in which
case I will advise you by e-mail concerning payment instructions.

If you are ordering a large-piece tray puzzle for special needs, there is an abbreviated order form at the
bottom of the Special Needs page for your convenience.

At this time, I lack the necessary Javascript skills to make the form truly interactive, i.e., to do the calculations
and all, but I have provided some OPTIONAL boxes on the right side of the form for your use in keeping track
of costs.  I suggest that, after you complete the form, you print out a copy for your records before
submitting the information to me.

After I receive your information, I will either contact you with any questions I may have, or I will e-mail you a
detailed, priced quotation with payment instructions.
